Was being used on two articles, Bill Clinton and John E. Arnold. For the former, it created an unnecessary notes section which was redundant given the fact that Clinton's name change is mentioned in the first article section about his early life. For Arnold, just the reference is enough. Really unnecessary to have a template just to make note of a person's name. The same could be said if this was used on articles about women who changed their last names after getting married.
