The Most Esteemed Perak Family Order of Sultan Azlan Shah ( Malay: Darjah Kerabat Sri Paduka Sultan Azlan Shah Perak Yang Amat Dihormati) is the second highest dynastic and chivalric order of the state of Perak which was constituted by Sultan Azlan Shah of Perak in 2000 to confer upon members of the royal family and eminent statesmen for their services to Sultan Azlan Shah during his reign as the Sultan of Perak.
The order was conferred in two grades of Superior Class and Ordinary Class. The order became dormant following the demise of Sultan Azlan Shah in 2014 and was replaced by the Perak Family Order of Sultan Nazrin Shah.
The Perak Family Order of Sultan Azlan Shah was conferred in two grades:
The first class or Superior Class (D.K.S.A.) was limited to senior members of the Perak royal family for their contributions during the reign of Sultan Azlan Shah. The second class or Ordinary Class (S.P.S.A.) was conferred upon eminent statesmen who held high positions and contributed immensely to Sultan Azlan Shah during his reign.

The Most Esteemed Perak Family Order of Sultan Azlan Shah ( Malay: Darjah Kerabat Sri Paduka Sultan Azlan Shah Perak Yang Amat Dihormati) is the second highest dynastic and chivalric order of the state of Perak which was constituted by Sultan Azlan Shah of Perak in 2000 to confer upon members of the royal family and eminent statesmen for their services to Sultan Azlan Shah during his reign as the Sultan of Perak.
The order was conferred in two grades of Superior Class and Ordinary Class. The order became dormant following the demise of Sultan Azlan Shah in 2014 and was replaced by the Perak Family Order of Sultan Nazrin Shah.

The first class or Superior Class (D.K.S.A.) was limited to senior members of the Perak royal family for their contributions during the reign of Sultan Azlan Shah. The second class or Ordinary Class (S.P.S.A.) was conferred upon eminent statesmen who held high positions and contributed immensely to Sultan Azlan Shah during his reign.
