Is "Ad" his given name and "Avis" his family name, or is "Ad Avis" his family name, with his given name unknown? Or, put another way, is "Ad" a common prefix to a last name, such as "bin", "van", "Mc", etc.?

It's never mentioned. When alphabetizing, always go with "ad avis" instead of "avis, ad"; he is never referred to as one or the other.
