Does this list mean these ISPs will take action against its users if it's documented they are a Wikipedia vandal/abuser? Or is it just a list where you can to look up each ISP's policy and decide whether to report a vandal or not? How receptive are ISPs to complaints of Wikipedia abuse? The Acceptable Use Policy at Qwest ISP has changed URLs.
New URL is http://www.qwest.com/legal/usagePolicy.html
