There are two parameters:

• logo: the name of the logo for that department, minus the File: prefix
• dept: the department name
• type: set to "sub" to change the wording to indicate that page is a subpage of a department

To add shortcuts, place them after the department name as additional parameters

Using {{Wikipedia:WikiProject U.S. Roads/Dept header |logo= USRD MTF.svg |dept=Maps |WP:USRD/MTF}} produces:
