why was it rejected?

I would like to add an awards bar (?) to represent the military awards of the subject I am writing about. I'm working on my first article and need some help. My first submission was declined on the basis of the tone not adequately formal and a need for more references. Also, I have issues with setting the copyrights on images of art work that I wish to upload.
Question #1: The review noted this text: "best known for his unique bas-relief woodcarvings" as unacceptable. I assume if I change that to "know for bas-relief woodcarvings" that that would be acceptable, yes?
Question #2: Is it possible someone could take a look at the article and give me further guidance in pointing out unacceptable language?
Question #3: I have a photographic portrait of the artist. As the heir of the artist I have the rights to this photograph. I would like to know how to upload it with the appropriate copyright.
Question #4: I have photographic copies of representative samples of the artist's work that I would like to upload as well. Again, as the artist's heir I own the original works. And again, I would like to know how to upload the photographic copies with the appropriate copyright.
