The problem with citation consistency is only an important issue when editing Featured articles, as they must maintain a consistent citation style. Most (but not all) of Wikipedia's medical FAs use the Diberri/Boghog format, with vancouver-style author names, and more than five authors truncated to three et al. It's VERY easy to generate a citation for those by plugging the PMID into this citation filling tool. My advice would be to use that tool on all articles, because it gives a much cleaner citation that creates less bulk in edit mode and is easier to edit around, but that's just my preference :) But you should always check the citation style on FAs, and stick to it.
So, the next thing is, how to know when you are editing a Featured article. There are many ways to check (see dementia with Lewy bodies):
