- The way I have it set out is with
{{Userboxtop|my username}}on one line and{{Userboxbottom}}underneath it, with the userboxes themselves in between. You can find all userboxes in Category:Userboxes or you can get examples from other users' pages. Just put the userbox title between two sets of double curly brackets and it'll show up on your page.
