I have assessed the article as B-Class and mid importance. There is some material that needs citations, but not enough to warrant anything lower than B-Class; however more references should be sought (an example is "A wrong-side failure of the signalling system, as a result of human intervention, was believed to be the cause"). Did the Train Driver 'Harold Joseph O'Neill' get convicted of the manslaughter charge? I have not been able to find a follow up source stating a conviction or cleared of charges

...a signal electrician was attending a failure of a track circuit... he forgot to remove the screwdriver to drop the track circuit relay when the section became occupied by a train, allowing a second train to see green signals and collide with the first... A Signals Electrician was also charged with manslaughter for his part in the incident.
