The 'fundamental causes' section looks to be arguing a case, going beyond the evidence actually given,and not necessarily doing everybody justice. A more modern enquiry might indeed look at defects of the system of management (if that was in its terms of reference) , but to do so it would take more evidence on these points:
and if it hadn't taken the evidence, it wouldn't leap to conclusions about what the evidence would have been. At the very least, the section should be reworked so that any leaping to conclusions is done by the reader.
